Notary services are available on a limited basis and by appointment only between 9 a.m. - 4 p.m. at Ronald Reagan UCLA Medical Center.
The Medical Center will notarize hospital business only.
The Medical Center will notarize Advance Healthcare Directives for patients.
There is no charge for either of these types of business (Advance Healthcare Directives or Hospital Business).
Those presenting documents for notarization must have the following identification:
- Valid California Driver's License or non-driver's ID issued by the Department of Motor Vehicles.
- U.S. Passport
- Foreign passport stamped by the U.S. Immigration and Naturalization Services (INS).
- Driver's License or non-driver's ID issued by another U.S. state. Photo must be incorporated.
- Driver's License officially issued in Mexico or Canada.
- U.S. Military ID.
- Inmate ID issued by the California Department of Corrections (but only to identify prisoners in custody).
- Resident Alien ID, or "Green Card," issued by the Immigration and Naturalization Service (INS), but only for notarization of INS forms. (Government Code, Section 8230).
