Transcranial Doppler assesses the blood flow velocity in the basal cerebral arteries. The Cerebral Blood Flow Laboratory employs the Medasonics Neuroguard TCD system, which has the capability for continuous monitoring during CO2 reactivity testing and embolus detection. Transcranial Doppler evaluations are indicated for the following:
- Ischemic stroke to identify impaired flow in the intracranial arteries
- Candidates for carotid endarterectomy to assess the cerebrovascular reserve,
- Patients harboring arteriovenous malformations to assess hemodynamics prior to and following treatment by embolization or surgery,
- Patients suffering traumatic and aneurysmal subarachnoid hemorrhage, who are at risk of developing cerebral vasospasm,
- Patients with traumatic brain injury to assess alterations in cerebral perfusion and intracranial pressure,
- Confirmation of the clinical diagnosis of brain death.
- Xenon-133 cerebral blood flow measurements are often correlated with transcranial Doppler findings to assess cerebral perfusion in:
- Cerebral vasospasm following aneurysmal or traumatic subarachnoid hemorrhage
- Arteriovenous malformation to assess autoregulation
- Cerebral vasospasm following aneurysmal or traumatic subarachnoid hemorrhage
- Arteriovenous malformation to assess autoregulation
