Palliative Care Consult Service 

Palliative care brings comfort and support to people with life-limiting illnesses. It offers relief from distressing symptoms such as pain, helps identify attainable goals of care, and focuses on the physical, emotional and spiritual needs of individuals and their families. Special attention is given to assuring comfort, dignity and the best possible quality of life.

Beginning with physician-to-physician clinical consultation, the Palliative Care Consult Service can help facilitate multidisciplinary services and expertise to help care for patients with advanced or terminal illness, and their families.

Appropriate Palliative Care Referrals

  • Setting attainable goals of care
  • Refractory symptoms, including pain, dyspnea, nausea, anxiety
  • Frequent admissions for the same diagnosis
  • Prolonged length of stay or in ICU with poor prognosis
  • Conflicts over medical decision-making
  • Help with determining hospice eligibility
  • Appropriateness of artificial feeding and hydration
  • Specialized care for those patients who are close to death and may not get home
