Are You A New Patient?

If you are a new patient making your first appointment with SMBP, please come to the office a full 15 minutes before your appointment time to fill out paperwork. If you have insurance please bring in your insurance card. You may also bring your medical records, x-rays or documents that you feel will be relevant.UCLA Health Santa Monica Bay Physicians.

Contacting your Physician

The standard charge for non-urgent medical advice from your physician is $25-$100.  Use this form to send a message to your practitioner.  if you do not receive a reply within 3 business days, please telephone your physician's office.

Messages you send to your doctor may be read by others in our practice.  Like phone calls, messages may be screened by office staff before being routed to the appropriate person for a response. While many subjects are appropriate for e-mail communication, it is ultimately you and your doctor's decision which information is appropriate for you. 

The following subject is never appropriate for e-mail:

  • Any urgent medical problem or emergency

Please keep in mind that although this messaging system can be a very effective tool, it is not a substitute for a physical exam or counseling by your doctor.
