Pre-Assigned Program

Pre-Assigned Program                                                                                                                 

Individuals who are personally invited to volunteer with UCLA Health by a supervisor/manager may enter through our pre-assigned volunteer program. Attendance at a pre-assigned session is required in place of general orientation. Currently, pre-assigned sessions are held on Tuesdays at 2 p.m. and Thursdays at 10 a.m. Sessions last approximately 1 hour and latecomers are not permitted.

Please keep in mind the following requirements:

Minimum length of volunteer service is one month, preferably longer.

 

Minimum age is 15

Letter of Invitation must be given to the volunteer to bring to the Pre-Assigned Session. Please click here to refer to the template to obtain required information for your letter of invitation.

 International Volunteer

After you have provided a letter of invitation to your prospective volunteer, they need to complete the following volunteer requirements, including ALL immunizations listed below, orientation materials and HIPAA BEFORE signing up for a pre-assigned session. All forms and training materials are listed below.  Please read over each section very carefully, as any missing forms, documents or immunizations will result in delayed processing of the volunteer.

Once all forms have been completed along with the required immunizations, the prospective volunteer may sign themselves up for a Pre-Assigned session here. All paperwork including the letter of invitation, copies of immunizations, and all forms printed from the website must be brought to the session.  Sessions are currently being held twice-weekly in the Westwood Volunteer Office.

ATTENTION HIGH SCHOOL STUDENTS:

IF YOU DO NOT HAVE A VALID DRIVER'S LICENSE OR PASSPORT, YOU WILL BE REQUIRED TO HAVE A GOVERMENT ISSUED ID, I.E., CALIFORNIA ID CARD TO OBTAIN YOUR BACKGROUND CHECK (DONE AFTER ORIENTATION). NO SCHOOL IDS ARE ACCEPTED. PLEASE APPLY FOR YOUR ID CARD AS SOON AS POSSIBLE IF YOU DO NOT HAVE A VALID DRIVER'S LICENSE OR PASSPORT.

Forms to Print, Complete and Bring with You to your Pre-Assigned Session Day:

Proof of Immunization Forms

Applicants MUST bring a copy of their immunization records to the pre-assigned session

You must show the following:

Measles, Mumps, and Rubella (MMR)

  • Medical documentation of TWO MMR vaccinations

     OR

  • Medical documentation of ONE BLOOD TITER indicating immunity to MMR
  • Disease history of MEASELS, MUMPS, and RUBELLA will not be accepted.

 Varicella (Chickenpox)

  • Medical documentation of TWO VARICELLA vaccinations

      OR

  • Medical documentation of ONE BLOOD TITER indicating immunity to VARICELLA
  • Disease history of CHICKENPOX will not be accepted.

Tuberculosis (TB) Screening

If you have always tested negative for TB, please provide:

  • Medical documentation of TWO SEPARATE TB skin tests, One TB test must be completed within the last 3 months. The other TB test must have been completed within the last 12 months. (NOTE: It's UCLA Health policy that you receive two separate TB skin tests and no exceptions will be granted.)

      OR

  • Medical documentation of a QuantiFERON Gold (also known as t-spot) blood test completed within the last 3 months.

If you have previously tested positive for TB, please provide:

  • Documented proof of a positive TB skin test or postive QuantiFERON Gold (also known as t-spot) blood test (documentation of postive TB from any date will be sufficient)

      AND

  • Medical documentation of a chest radiograph (chest x-ray) report within the last 3 months.

Tetanus, Diphtheria, Pertussis Vaccine (Tdap)

  • Medical documentation of Tdap vaccination

      OR

  • Signed Tdap declination if unable to provide medical documentation.

Our department only processes volunteers for Ronald Reagan UCLA Medical Center. Those interested in working at Santa Monica UCLA Medical Center need to contact Elaine Eastwood at (424) 259-8176. Volunteers who will be working in a research lab or department through the School of Medicine and DO NOT have patient contact or access to patient information or perform any work within a patient care area, should be processed through the David Geffen School of Medicine.
