Requesting Hours Verification from Volunteer Services
Anyone who has volunteered at least 100 hours in their respective department may request hours verification from Volunteer Services. Please click here to download the Hours Request Form. Once you have completed the form, please return to the Volunteer Services Department, or fax to the number listed below. Please allow two weeks for processing.
