Become a Patient

In order for our team to best understand your medical needs, we need complete and current medical record information. This way, a coordinator can match your needs with the most appropriate specialist. Specialists review your medical records to help determine if we can offer a particular type of advanced or alternative treatment.

Referral Process

An International Patient Coordinator will facilitate the following process of becoming a patient at UCLA Health.

1. Contact the UCLA Health International Office

2. Send Medical Records

Send complete and current (past three months) medical records to UCLA Health International including copies of any relevant radiology scans and test results in English. 

3. Obtain Medical Acceptance

A UCLA Health physician may review your medical history. If treatment is recommended, a preliminary treatment option may be determined.

4. Obtain Financial Clearance

You will need to provide either an advance deposit of estimated charges or your insurance company must provide an authorization approved by UCLA Health that guarantees payment. Insurance authorizations can only be accepted for consideration through firms that have verified contracts with UCLA Health.

5. Schedule Appointment

An International Patient Coordinator will help you schedule an appointment at a convenient time with an appropriate UCLA Health physician. A coordinator will help you complete the necessary forms for appointment scheduling and registration. You can submit the Request an Appointment Form or call us at +1 (310) 794-8759.

An International Patient Coordinator remains at your service from the moment you contact us and throughout your stay to manage any medical, financial, or service-related needs or inquiries that must be addressed.
