Memorial and Tribute Gifts
Memorial Gifts
In lieu of flowers, families may request that memorial gifts be directed to support patient care, medical education, or research.
For the family of the deceased: Supply the UCLA Health Sciences Development office with the contact information of the family member to be notified of the memorial gifts. The development office will notify the designated family member of these gifts and forward the names and addresses of the donors.

When writing an obituary, include:
"In lieu of flowers, memorial gifts to support {optional designation} can be made to The UCLA Foundation {or The UC Regents, depending on designation}, UCLA Health Sciences Development, 10945 Le Conte Avenue, Suite 3132, Los Angeles, California 90095-1784 or at www.uclahealth.org/giving; indicate in the check memo or the online comment section that the gift is designated to {optional designation}."
Tribute Gifts
Your gift to UCLA can be a thoughtful way to recognize or honor a physician or member of your healthcare team, a special occasion, or a loved one.
You can make these donations online or mail your contribution, including the details regarding the person(s) being honored, to the address below. As a follow-up, we can inform the person(s) for whom the gift was made. Please include details and contact information in any correspondence.
