For Our Patients
At the UCLA Family Health Center, we strive to offer you excellent service throughout your care, starting with your check-in at our front desk.
If you are a new patient, we will ask you to fill out forms that will provide us with information about you and your health history. And because we’ll be starting what we hope to be a long relationship, we’ll also need to collect your address and contact information, as well as information about any medical insurance plans you may have.
