Medical Care Coordination (MCC) is a program that combines your medical and psychosocial support in one place. The MCC team will work with you and your physician to determine your needs and goals related to your health. If you are having trouble taking your medications, getting to your appointments, keeping track of your other medical appointments, having enough food, or finding someone to talk to, we can help.

If you are interested in having your medical and psychosocial support services tied to your HIV medical care, please ask your HIV provider to refer you to the MCC program. We also welcome self-referrals. To self-refer, please CLICK HERE and fill in the requested information. We can provide resources for patients that are not eligible for the program.
