Care Extender Preferences

Don't miss out on your chance to choose your Winter 2024 Rotation Preferences! The submission window will open on Sunday, March 3rd at 8:00am PST and close on Sunday, March 17th by 11:55PM PST. Click on the link to submit your department preference for the Winter 2024 rotation. 

Please read the instructions in their entirety before signing up for your preferences.

Please note that any incorrect or late submissions will result in your assignment into any department (RRH or SMH) that has available space. No reassignments will be made for missed deadlines. Please be sure to read the department descriptions below and the instructions before choosing your preferences.

For departments that have a note about mandatory training, you should only sign up for that department if you are able to attend the training day noted. If you sign up incorrectly or do not submit your preferences, you may be assigned to a random department where we have space, and no reassignment will be made. You are required to complete this form, even if you are planning to take a full rotation LOA, inactivate/complete the program.

Department Preference Survey

To submit your preferences form, please enter the code displayed on the confirmation screen of the main Department/DC Evaluations. Your feedback on these surveys is valuable in helping us improve both our program and individual departments. Kindly note that these surveys relate to the department where you volunteered previously and not your current ones.

  • If you are currently a first rotation CE: please disregard these surveys, you will not be prompted to enter a code when submitting your preferences.
  • If you are currently a second rotation CE, please fill out both surveys.
  • If you are currently in your third rotation and on, please fill out ONLY Main Dept/DC Eval. 

You may submit your evaluations now in order to receive your code so that you may submit your preferences once it opens.
