The Department of Care Coordination and Clinical Social Work was established in 2004 and serves all patients within the hospital system as well as pediatric, oncology, and transplant patients in UCLA Westwood Clinics.

Patient and Social Worker

Scope of Services

RN Case Managers and Clinical Social Workers in the Department of Care Coordination and Clinical Social Work at UCLA Health are available to serve patients with any diagnosis (and their families) referred from:

  • All in-patient adult and pediatric medical, surgical, transplant, and rehabilitation services
  • The Emergency Department
  • All in-patient or Emergency Department.
  • All in-patient or Emergency Department elder/dependent adult abuse/neglect referrals
  • All in-patient or Emergency Department adult domestic violence referrals

Support Groups

UCLA Health and its community partners offer a number of support groups, available in person and by telephone, designed to support and inform patients, families and caregivers coping with a variety of diagnoses, including Alzheimer's and dementia, heart disease, and others.
