Do patients have the right to request an amendment or addendum to their medical information?

Yes. If a patient feels that medical information contained in their record is incorrect or incomplete, they may ask us to amend the information or make an addition (addendum) to the record. The requests for amendment and/or addendum must be made in writing to the address below, and include the reasons that support the request.
