What should I do if I need to request electronic Restricted Information (which includes PHI) for my research project?

In order to obtain the information, you must complete and submit a Request to Interface or Download Restricted Information form. The completed form must be submitted to the Office of Compliance Services – Information Services and Privacy for review along with the following documents:

  1. Initial IRB application (HS-1)
  2. Final IRB approval letter
  3. Any data collection tools you will be using (Excel, Word, etc)

All the data you are requesting access to must be identified in your initial IRB application and data collection tool(s). Any discrepancies may result in a delay or denial of your request.
