Mentoring Form
To be used as a guide during mentee/mentor meetings for discussion; it does NOT need to be filled in, signed or turned over to anyone. Mentors are recommended to keep discussion notes for future meetings with mentees.
Mentor Confirmation Form
To be completed and signed by both mentee and mentor during semi-annual meeting and returned to Leticia Gutierrez in the division office; will be filed in the mentee's personnel file to confirm active participation in Career Development Mentoring Program.
Mentor Evaluation Form
To be completed and signed by the mentee during semi-annual meeting and returned to Leticia Gutierrez in the division office; will be filed in the mentor's personnel file for their career advancement reviews as necessary.
Career Planning Conference Form
This form is FYI only.
It will be distributed to faculty under consideration for advancement by the division personnel office.
