How To Incorporate the Functional Status Template on the H&P
On Care Connect, click on SmartPhrase Manager. The SmartPhrase Selection window will appear. On this window, a) under User, enter Cook, Erin Atkinson, and b) under Phrase, enter ADLS, then click on Go right next to it. The SmartPhrase Editor window will appear. Click on Owners & Users tab, and in this section on the R lower corner, click on Add Myself and then Accept right below it. This will add the Functional Status template to your My SmartPhrases under the name ADLS. To incorporate this Functional Status template on your H&P, enter dot ADLS (.adls)

How to Use the Discharge to Facility Navigator on Care Connect
On the Encounter:

Click on Discharge Tab (left column)

Click on Discharge to Facility (3rd tab on the new window)

Click on Rx Routing
-
Click on No Print option under Default Order Class

Click on Next (R lower corner)

Open DC Order Set
-
Click on 1) Review Orders for Discharge
Orders needing review will appear.

Go over each medication and click on the appropriate action: refill, modify, prescribe, stop taking, or resume.
Make sure each medication to be continued has complete dosing instructions. If it includes an antibiotic, the stop date must be indicated.
Once this is completed, the right column window "Orders Need Reconciliation" would turn green.

Click Next

Place New Orders
In this window: enter any medication the patient needs to be on, that was not included in the above list. Important examples that are not to be missed:

New antibiotics that you plan to add based on the sensitivity results that just became available, or based on new ID recommendations

Coumadin that may have been held due to previously supratherapeutic INR, but which needs to be restarted

Click Next

Under Order Sets and Pathways Click on box Discharge to Facility IP under Suggestions. An order set appropriate for transfers to SNF will appear; click on all the applicable instructions.

Click Next

Click Review and Sign

Click on Pend (or Sign, if discharge will occur within the next 2 hours)
Note: On weekdays: Pend the orders and please inform the Geriatric Unit pharmacist Dominick Bailey (P 91147) to review the discharge medications. After he has reviewed the discharge medications and the orders have been finalized, Sign.
