As of Monday, January 12, 2020, all contracted and temporary staffing requests will be made through the web-based Vizient portal.
Vizient Staffing is our new centralized vendor management system that will handle all of UCLA Health’s contract labor management services. This includes Nursing and Allied Health temporary/registry personnel. Our organization’s focus in partnering with Vizient will be to enhance our ability to strategically source vendors, provide reporting and utilization analytics, and develop streamlined vendor management processes to ensure that we meet the registry needs of those we serve.

JotForm – Contractor Request Form
1. “How do I request a new long term contract/travel candidate?” (Similar to, “Where do I get the Jotform link”?)

a. Please use this Jotform link to request all contract labor staff after January 12, 2020, through Vizient’s program
b. After the request is approved by your position/budgetary approver, it will go to Vizient’s team for uploading into their Optimizer technology and out to the approved vendor panel for recruitment.
2. “Who is my position approver on the Jotform?”
a. This would be the Executive Director or person who was previously approving your temporary staffing positions or whomever has budgetary approval for your department. You no longer need to submit a requisition request through Cornerstone. The Jotform has replaced the requisition request in Cornerstone.
b. This portion of the form just ensures there is position control for contracted roles within the new process for UCLA and Vizient.
3. “How do I make an extension request when a current contractor is nearing the end of their assignment?”
a. When the time comes for your candidate to be extended, please use the Jotform link and click “Yes” on “Is this an extension Request?”

b. You will answer a series of questions and complete the Jotform as you would a new contractor request and get position approver approval.
c. It is best practice to keep our UCLAStaffing@vizientinc.com team involved as soon as you know you may want to extend a candidate, especially from their direct UCLA agreement to Vizient’s new program.
i. Once approved, Vizient will want to make sure assignment details are gathered, compliance has not lapsed, and that the rates are close from one contract to the other.
ii. Vizient will also want time to communicate expectations with the vendors throughout that entire process to make sure the transition is smooth.
4. “If I am in the middle of my existing assignment and want to transition them to Vizient’s program, what do I do?” (This has come up a few times with assignment lengths that are in excess of 4 months long and recently renewed prior to Jan. 12 Go-Live)
a. Before requesting anything in Jotform, please communicate your desire with both Vizient’s team (UCLAstaffing@vizientinc.com), Colin Walsh (walsh@vizientinc.com), and UCLA’s contracting team (Mark Sampson and Victor Banh).
b. These scenarios will be handled on a case-by-case basis and we’ll want to make sure all parties are on the same page making the decision on how to move forward.
5. “What do I do if I do not see my cost center and/or department in the Jotform?”
a. Vizient did a lot of data collection work with UCLA’s teams, but not everything may have been captured.
b. First, please check with our UCLAStaffing@vizientinc.com team to see if it was labeled in a different manner than you are accustomed.
c. If it was not captured, we have a Cost Center Request Form that was created by UCLA and Vizient’s team to add these to the Jotform and to our Optimizer technology.
