The UCLA Adolescent Transitional Cardiac Care Program provides comprehensive management for adolescents and young adults with heart disease, ages 12 to 21.
Cardiac Services
- Diagnostic facilities
- Cardiac and medical management
- Therapeutic interventional cardiac catheterization and electrophysiological facilities
- Surgical management
- Heart and lung transplantation
Transitional Services
- Provides adolescent with self-management skills necessary to navigate the health system
- Assists teens to understand their condition and develop a life plan to identify educational, social and career goals
- Provides support for parents in the transition process

Family Support Services
There are a variety of family support services and groups available to parents and families of teens with heart disease including educational meetings scheduled throughout the year.
