Who do we send the completed form to?
This form gets sent to the department payroll representative.
Question:
On the form there is a question if PAR was sent, what does this mean or what category to use?
This is for clinical staff who initially would be CN I and transition to CN II for PAR (promotion for new grads).
Question:
If an employee was precepted by 2 Preceptors, is there an option to split the award?
2 preceptors are allowed, both names must be on the form and the amount is split evenly.
