Teach Back MedUcation is the UCLA method of patient education on the topic of medication. MedUcation with Teach Back is done to insure patients understand their medications, including the purpose and side effects of the medications they are prescribed. We want to reduce the risks of readmission related to home medication management through education. The Teach Back method of patient education is an evidence based practice proven to improve understanding, increase retention of information and reduce readmission rates.
At UCLA Health we have implemented the use of medication cards for commonly ordered medications. Individualized medication cards are given to a patient when a new medication is ordered. Nursing then uses the cards to discuss the purpose, side effects, dosing schedule and special considerations of each medication.
MedUcation is how we are helping our patients have a better understanding of their medications so they manage their medications more safely when they go home.
