New Patients

If you are a new patient making your first appointment, please come to the office a full 15 minutes before your appointment time to fill out paperwork. If you have insurance please bring in your insurance card. You may also bring your medical records, x-rays or documents that you feel will be relevant.

To create the safest environment possible for children, our office follows the vaccine schedule recommended by the American Academy of Pediatrics. We refer patients to other offices if parents do not wish to vaccinate their children or wish to significantly delay immunizations.
