You can complete the Universal Consent process at any of the locations listed below. The process includes watching a 5 minute informational consent video and answering two questions. Additional locations will be available in the near future. You will need your medical record number to complete the consent.

The Universal Consent is also offered via a message on the myUCLAhealth electronic record patient portal and appointment text reminders. Patients who have an active myUCLAhealth account may receive a message in their portal prior to an upcoming appointment. Patients who are set up to receive appointment text reminders may receive a link in the text that will allow them to complete the Universal Consent.
