Make the most of your first visit with us! Gather the following information to help us help you:

  • Past medical and surgical history
  • Your daily medications
  • How all of the organs of your pelvis are working
  • Your sex life, specifically any pain or discomfort you experience during or after intercourse

Below you will find our new patient questionnaires. If possible, please complete and bring with you to your first appointment with us. The more information we have about you, the better we can diagnose and develop a customized treatment plan for your symptoms.
