At UCLA Health - Santa Monica Medical Center, UCLA Geriatricians provide holistic hospital geriatric care for older patients on a special Geriatric Medicine unit with dedicated staff and resources.
These staff members include:
- faculty geriatricians and UCLA resident physicians in training
- nurses with special training in caring for the geriatric population
- physical, occupational and speech therapists
- pharmacists
- dietitians
- social workers
- case managers
- spiritual care providers

Our case management team of social workers and case managers assist in short- and long-term care planning. When patients are admitted to our inpatient service, the geriatric faculty communicates with their primary care physicians on a daily basis. We coordinate care through the various transitions that follow hospitalization, including transitioning to post-hospital care rehabilitation facilities, if indicated.
In addition, we offer consultative care for elderly persons hospitalized at Ronald Reagan UCLA Medical Center in Westwood and Resnick Neuropsychiatric Hospital at UCLA.
