UCLA Health notifies patients about activity involving protected health information

UCLA Health identified activity involving patient information that was inconsistent with the institution’s privacy policies governing protected health information. In some instances, certain patient information was also disclosed to an outside health care provider.

Upon discovering the activity on July 2, 2026, UCLA Health promptly took steps to stop the activity and initiated an investigation. At this time, UCLA Health has not identified any evidence that the information has been further disclosed or otherwise misused as a result of this incident.

The information involved varies by individual and may have included patient names, addresses, dates of birth, and health insurance information, as well as clinical information such as referral orders. For some individuals, the affected information may have also included the last four digits of a Social Security number. No full Social Security numbers, financial account numbers, or payment card information were involved. 

Administrative and technical safeguards have been reviewed, and the institution is implementing additional measures to further strengthen oversight of access to protected health information.

UCLA Health is notifying potentially affected individuals and appropriate state and federal agencies. Patients are encouraged to remain vigilant by reviewing explanations of benefits, health insurance statements and other account statements for any unusual activity.
