To request a Verification of Hours ONLY, please complete a Request for Hours form and select "Verification" and the delivery method (mail or email). Submit the completed and signed form to your designated program coordinator:
NOTE: Due to the COVID-19 pause, if you were affected and could not complete the minimum 100 hours commitment requirement, we will be able to verify your hours still.
To request a Certificate of Completion, please off-board with your program coordinator first. Volunteers must "complete" the program with a minimum of 100 hours recorded in order to be eligible for the certificate of completion. Then, complete the Request for Hours form and select "Certificate" (you may also select "Verification" if you would like both) and the delivery method (Certificates cannot be sent by email, only mail).
NOTE: Due to COVID-19 pause, certificates will not be mailed out until onsite operations resume. Certificates can take 1-2 weeks or more to process.
The UCLA Health Sciences Volunteer program was also previously known as the Direct Referral Program.
If you would like to request your hours from your previous role in the Direct Referral Program or you recently off-boarded from the UCLA Health Sciences Volunteer Program, please complete the above instructions. Please note, the rules still apply and certificates cannot be issued if you did not off-board properly.
NOTE: our database system does not keep inactive records that are more than 5 years old, please request your hours as soon as you off-board from the volunteer program or as soon as possible before the 5-year mark.
