To make an appointment with one of our dermatology specialists, please call us at your nearest location.

Once you have scheduled an appointment with us, it is important that you please remember to bring the following items with you on the day of your appointment:

  • A copy of your insurance card showing your policy number, group or certificate number. If possible, please bring a copy of your insurance billing form.
  • A current government or state issued photo ID (such as a military identification card, a driver's license, passport, border crossing card, etc.), or a school issued photo ID.
  • Your Medicare or Medi-Cal card (if applicable).
  • A complete list of any medications, vitamins, or supplements that you are currently taking.

Please arrive 15 minutes early to fill out the new patient packet.
