The UCLA Comprehensive Pain Centers warmly welcome new patient referrals, and same-day appointments are often available. We offer consultation services for patients with challenging pain conditions, whether the pain is acute or chronic. We also offer consultation services for patients with chronic pain conditions who need to undergo surgery, so that the plans for their care by physician anesthesiologists who specialize in pain medicine may be coordinated before, during, and after their procedures.

For physicians outside the UCLA Health, a consult request may be telephoned or faxed to any of our clinic locations in Burbank, Encino, Santa Clarita, Santa Monica, Thousand Oaks, Torrance and Westwood, whichever is most convenient for the patient.

  • Patient information (demographics, face sheet)
  • Reason for referral
  • Recent clinic notes
  • Results of any diagnostic studies
  • Authorization information (if required)

Within UCLA Health

Within the UCLA system, a patient may be referred either for a chronic pain consultation or for a pre-surgical pain consultation, using the CareConnect electronic health record. To enter an order for a pain consultation, please follow the instructions below. Please call and speak with the staff at any of our clinic locations if you have questions, and they will be happy to assist you. Same-day appointments may be arranged for urgent pain issues.

Chronic pain consultation

To refer a patient for chronic pain consultation, first open an ambulatory encounter. Within "Orders", open the Preference List for "Pain", and click on "Referral to Pain Mgmt".

In the window for "Referral to Pain Mgmt", select “Internal Referral”, and click on the appropriate clinic location. Complete additional information as appropriate. The most common referral is for general evaluation and management. A specific procedure may be requested if desired for a patient with an established diagnosis.

Pre-surgical pain consultation

To refer a patient for pre-surgical pain consultation, first open an ambulatory encounter. Within "Orders", open the Preference List for "Pain", and click on "Referral to Perioperative Pain Mgmt".

In the window for "Referral to Perioperative Pain Mgmt, select "Internal Referral", and "PAIN CLINIC MP2". Complete additional information as appropriate, including the planned surgical procedure and the scheduled date of the surgery.
