What is the UCLA Health Assessment Program?

The UCLA Health Assessment Program aims to help you prepare for your annual wellness visit with your primary care physician. During the visit, a nurse practitioner updates your health history, reviews your medications and completes any necessary health screenings in collaboration with your primary care physician.

These appointments are available to UCLA Health patients with Medicare Advantage HMO plans. They’re designed to help ensure that you make the most of your time with your primary care physician.

Accessible, affordable care: We offer health screening appointments virtually or in-home. These voluntary appointments are covered by your Medicare Advantage plan.

Comprehensive health management: The appointment allows for extra time to discuss your health concerns and management strategies in depth. A nurse practitioner coordinates with your primary care physician before the visit to ensure they adequately address your needs.

Preventive health services: Meeting with our nurse practitioner helps you to be proactive rather than reactive with your health. The nurse practitioner assesses your risk factors and gives personalized recommendations to improve your well-being. 

What happens during a health assessment visit?

A UCLA Health patient service representative will call to schedule your Medicare Advantage health assessment visit. They typically schedule it one to two weeks before your annual wellness visit with your primary care physician. You meet with a board-certified nurse practitioner to discuss your:

  • Overall health goals
  • Advance directive
  • Health screenings
  • Lifestyle habits
  • Medications
  • Ongoing health concerns
  • Vaccinations

You get valuable one-on-one time with a nurse practitioner for a 30- to 60-minute visit in the comfort of your home.

Throughout the visit, the nurse practitioner may identify areas of care that need to be addressed. They may recommend lifestyle changes to improve your health, help manage chronic diseases, recommend preventive screenings or refer you to other UCLA Health specialists.

What happens after a health assessment visit?

Communication is a vital component of the UCLA Health Assessment Program. After the visit, your nurse practitioner communicates the assessment results with your primary care physician. This provides a clear, focused understanding of what to discuss with your primary care physician during your upcoming annual wellness visit.

You have your annual Medicare wellness visit one to two weeks after the health assessment visit. Your health assessment visit does not replace your annual wellness visit.
