           You will be followed throughout your stay by a UCLA Health physician, with visits averaging 1-3 times per week.

• Seamless Access to Your Medical Information

           Your physician has up-to-date access to your hospital records, imaging, laboratory results, and care plan- ensuring continuity and accuracy. 

• Coordinated Communication with Your Care Team

          Your UCLA physician maintains close communication with your primary care provider and any UCLA specialists to keep everyone aligned on your treatment plan. 

• Assistance with Specialist Appointments

          All necessary follow-up appointments with UCLA specialists will be arranged during your stay, making the transition to outpatient care smooth and convenient. 

• Integrated Medical Records

          Records from your Skilled Nursing Facility stay will be included in your UCLA electronic medical record, providing your UCLA outpatient care team with seamless access to your health information to support your ongoing care.
