To Refer a patient or receive answers

How to Refer a Patient for Transplant Evaluation at UCLA
The patient, the patient’s physician, the patient’s medical group or the patient’s dialysis unit can make the referral to UCLA transplant programs by calling the appropriate referral number above.
Referring physicians will be promptly contacted by a physician on the transplant team to discuss their patient's case, and an appointment will be scheduled. For new patients: Please fill out and send this referral form to us by fax at 310-983-3620.
