Welcome to UCLA HealthLink for Medicare Advantage Providers
HealthLink provides secure, real-time access to network providers, vendors, and office staff to securely connect to UCLA Health Medicare Advantage Plan’s EHR system to improve continuity of care, check managed care eligibility, submit and review referrals, and view claim status.
Any computer with a broadband internet connection, appropriate security, and the required hardware can access UCLA HealthLink quickly and securely.

Complex Care Management for UCLA Medicare Advantage Members
We understand that managing patients with multiple chronic conditions can be time consuming and complex. Our Complex Care Management (CCM) team is here to support you and your UCLA Medicare Advantage patients by providing an extra layer of clinical and social support at no additional cost.
Our team will collaborate with you, other providers on the care team, the patient, and caregivers to build a customized care plan that addresses the patient’s clinical needs. We aim to improve health outcomes, reduce avoidable hospitalizations, and enhance the overall patient experience.
How does the Program Work?
Patients are referred to Complex Care Management through various channels including primary care providers, specialists, recent hospital or emergency room discharges, health risk assessment (HRA) completion, or self-referral.
Once enrolled, our care team:
- Conducts a comprehensive assessment (including clinical, behavioral, and social domains)
- Develops an individualized care plan aligned with patient preferences and clinical goals
- Provides regular follow-ups and care coordination
- Engages caregivers and family members as appropriate
- Works closely with the provider team to ensure timely communication and alignment of care efforts
How we support Providers
Our team is here to complement your care by offering:
- Coordination of services (e.g., home health, DME, behavioral health)
- Medication reconciliation and adherence support
- Patient education and coaching
- Help addressing social determinants of health (SDoH)
Referring a Patient to Complex Care Management
If you have a UCLA Medicare Advantage patient who may benefit from Complex Care Management, we welcome your referral. Ideal candidates include those with:
- Multiple chronic and poorly controlled conditions
- Frequent hospital or emergency room use
- Functional or cognitive decline
- Polypharmacy or difficulty managing medications
- Limited social support or unmet social needs
