Ways to Complete the Universal Consent

We make it easy, safe and free to complete the Universal Consent and allow researchers to use your leftover biological samples. It’s a one-time process that takes around eight minutes and involves watching a short video.

You can complete the consent online at any time, with instructions in English, Spanish, Farsi, Mandarin, Korean or Arabic. You can also do so during a medical appointment or inpatient stay at certain UCLA Health locations. We may send you reminders, too.

Consent through the MyChart portal at myUCLAhealth

You can complete your consent at myUCLAhealth if you have an account set up. The form is listed under the “Questionnaires” tab for both mobile and desktop. It’s available 24/7.

If you need to, you can pause while completing the form and return to it later. You may see requests to fill out your consent in your MyChart appointment reminders. You may also see a similar reminder message on the home page of your patient portal.

Consent through text message appointment reminders

Some people choose to receive text message reminders of their UCLA Health appointments. If you have done so, you may see a link to the consent process at the bottom of the message. You can follow this link when it’s convenient. You will need to enter your cell phone number for verification.

Consent during an inpatient stay

People who wish to consent during an inpatient stay at UCLA Health can use the bedside iPads available in each room. The form is available through MyChart, though the interface looks a little different than it does on your own computer or mobile device. Look for the consent form under the tab labeled “a few questions.”

Consent during an outpatient appointment

A number of UCLA Health locations and affiliated partners offer the opportunity to complete your consent while you wait. Some of these locations may feature trained volunteers who can answer questions, troubleshoot and help you navigate the consent process.

We offer two ways to consent during outpatient appointments. You will need your UCLA Health medical record number (MRN) to either:

  • Scan a QR code: Front desk staff will provide a flyer about the program that contains a QR code. The camera on your mobile device scans the code and takes you to the online consent.
  • Use an iPad: Staff can also provide an iPad with a link to the consent form. Staff disinfect these devices after each use. Unfortunately, there’s no way to save your progress if your appointment begins before you finish.
