Eligibility Requirements
Applicants must meet the following requirements by the residency appointment date:
- Graduation from a Liaison Committee for Medical Education (LCME)-accredited medical school in the United States or Canada
- Satisfactory completion of an ACGME approved internship in medicine or surgery
- Satisfactory completion of the USMLE, Part 3
Graduates of foreign medical schools may be considered for residency appointments with proper documentation from:
Educational Commission for Foreign Medical Graduates
ECFMG J-1 clinical visa required. More information is available through the David Geffen School of Medicine Visa and Licensing Office
Medical Board of California
PTAL or California Medical License required.
Applicants who are not United States citizens should refer to the David Geffen School of Medicine Visa and Licensing Office about UCLA-sponsored visas.
Application Process
Applicants to the Ophthalmology Residency Program at the David Geffen School of Medicine at UCLA must apply through . Only applications received directly from SF Match will be considered. Deadline to submit applications is September 30.
Applicants to the EyeSTAR Program must submit a supplemental EyeSTAR Program Application along with three EyeSTAR Program Evaluation Forms and letters of recommendation. These should be sent directly to the Stein Eye Institute EyeSTAR Program following the instructions on the forms. The program administrator must receive all supplemental application materials by September 1.

- PERSONAL STATEMENT: Summarize the research you have done, including your background, interests, special accomplishments, significant findings and the research area you wish to pursue in the STAR program. Discuss the role you expect research to have in your long-term career. Indicate any Ph.D. departments you wish to consider.
- TRANSCRIPTS: Medical, Graduate, Undergraduate (if available). Copies of official transcripts are acceptable.
- SCORES: ECFMG, GRE, MCAT, NBME, other scores (as applicable). Copies of official scores are acceptable.
- CURRICULUM VITAE and REPRINTS OF PUBLICATIONS, if any.
- THREE LETTERS OF RECOMMENDATION: Please use the enclosed forms. Forms and letters are to be mailed by your evaluators. One of these letters should be from the faculty member with whom you have most recently done extensive research. He/She should address your potential for future research contributions.
