Copies of your medical record can be obtained by contacting the Medical Records Release of Information customer service desk. The Medical Records Release of Information and Customer Service for Resnick Neuropsychiatric Hospital (RNPH) and Behavior Health Services Office is located inside the Jules Stein Building at 100 Stein Medical Plaza, Room BH-239A.

You can request a copy of your medical records by completing the Authorization for Release of (PHI) Protected Health Information form found by clicking here.

Directions

Walking directions from Ronald Regan UCLA Hospital/Medical Center:

  • Walk to the front of Ronald Regan Hospital
  • Go to the Signal in front of the Hospital and Cross Westwood Blvd. 
    • After you cross the street, make a right and walk to the Orange Sign for 100 Stein Medical Plaza and make a quick left and follow the walkway to Jules Stein.
  • Enter into the Jules Stein Building Lobby.  
  • Once you enter the Jules Stein Lobby, continue walking straight down a long hallway.  
  • At the end of the hallway, there will be 2 white double doors.  
  • Follow the instructions on the doors to gain entry into RNPH Medical Records.

               

Walking directions from 100, 200 and 300 Westwood Blvd. Medical Plaza:

  • Exit to the front of the Building and walk to Westwood Bvld.
  • Go to the first Signal in front of the Buildings on Westwood Blvd and cross the street.
  • After you cross the street, make a left and walk to the Orange Sign for Jules Stein 100 Stein Plaza and follow the walkway to the Jules Stein Building(Stein Eye Institute).
  • Enter into the Jules Stein Building Lobby.  
  • Once you enter the Jules Stein Lobby, continue walking straight down a long hallway.  
  • At the end of the hallway, there will be 2 white double doors.  
  • Follow the instructions on the doors to gain entry into RNPH Medical Records.
