The award-winning UCLA Alzheimer’s and Dementia Care (ADC) Program is designed to help patients and their families with the complex medical, behavioral and social needs of Alzheimer's disease and other types of dementia. Our Dementia Care Specialists are at the heart of the program and work with the patient’s primary care doctor/specialist to create and implement a personalized care plan. The UCLA ADC Program provides comprehensive, coordinated care for persons with Alzheimer’s and related dementias and their loved ones.
Our Dementia Care Specialists are nationally recognized Nurse Practitioners, with advanced knowledge in caring for persons with dementia.
We begin with a 90-minute evaluation, a personalized care plan developed with the referring physician, and follow-up phone calls or in-person visits to make sure the plan is implemented or modified as needed.
