The UCLA ADC Program provides comprehensive, coordinated care for persons with Alzheimer’s and related dementias and their loved ones.
Expertise
Our Dementia Care Specialists are nationally recognized Nurse Practitioners, with advanced knowledge in caring for persons with dementia.
Personalized Care
We begin with a 90-minute evaluation, a personalized care plan developed with the referring physician, and follow-up phone calls or in-person visits to make sure the plan is implemented or modified as needed.
24/7, 365 Access
Caregivers and loved ones who need assistance or advise have 24/7, 365 day a year access, in order to avoid Emergency Department visits and hospitalizations.
