Provider Resources

The UCLA Alzheimer's and Dementia Care Program provides a variety of services for established patients and caregivers, which include the following: 

  • Caregiver education regarding disease progression, management of behavioral disturbances 
  • Caregiver Support 
  • Advance Care Planning 
  • Referrals to neurology, psychiatry, or geriatrics 
  • Referrals to community-based organizations 
  • 24/7 access  

Patient Eligibility 

For your patient to be eligible for the UCLA Alzheimer's and Dementia Care Program, the patient must meet the following criteria:

  • Must be referred by a UCLA Primary Care Physician, Neurologist, or Psychiatrist who is willing to collaborate with the Dementia Care Specialist 
  • Have a confirmed diagnosis of Alzheimer's or other types of dementia
  • Have a caregiver(s), preferably a family member, who is willing to participate in care  
  • Not reside in a skilled nursing facility or be enrolled in hospice care

How to Enroll Your Patient 

If you are a UCLA Primary Care Physician, Neurologist, or Psychiatrist and you would like to enroll your patient in the program, please place a referral on Care Connect to the ADC Program.
