Application File Requirements

Please submit the following to the SF Match application database at sfmatch.org.

Completed Fellowship Application

• Central Application Service (CAS) Application
  • Complete Personal Info form
  • Submit Required Supplemental form
• Personal Statement *
  • * Please refer to the CAS Applicant Instructions for details.
• Copy of Medical School Diploma (or letter from medical school with official seal verifying date of graduation; international medical graduates should include a notarized English translation)
• USMLE Steps 1, 2, and 3/COMLEX 1, 2, and 3 or equivalent transcripts.
• ABA BASIC Exam Scores
• Three letters of recommendation by physicians who are familiar with you professionally and personally**
  • ** One Letter of Reference must come from the anesthesiology residency program director.
• Dean's Letter/Medical Student Performance Evaluation
• In-Training Examination Score Reports during residency 

Please submit the following required supplemental documents to [email protected]. 

• Curriculum vitae including a list of publications
• Copy of Medical School Diploma (or letter from medical school with official seal verifying date of graduation; international medical graduates should include a notarized English translation)
• Copy of Valid ECFMG Certificate (if international medical graduate)
• Copy of ABA Basic Examination Result Letter
• Wallet-sized Photo
• Copy of Valid State of California Medical License (if practicing in California)
• Copy of DEA License (if practicing)
• AAMC ID number
