The Extensivist Program is designed to help you better understand and manage your medical conditions. Our extensivist team will help you navigate the healthcare system and coordinate care with your primary care doctor and specialists. You may benefit from our program if you have been admitted to the hospital more than once in the past year, and have one or more chronic medical conditions such as:
- Congestive heart failure
- Chronic obstructive pulmonary disease
- Cancer
- Diabetes
- Prior stroke
