Dementia Care

Referring Providers

The UCLA Alzheimer's and Dementia Care program follows a co-management model that works closely with the primary care and/or referring physician to insure that care is comprehensive and coordinated by:

  • Maintaining and expanding a dementia registry
  • Conducting structured caregiver/care-recipient needs assessments of patients in the registry
  • Creating and implementing individualized dementia care plans based on needs assessments that can include:
    • Consultation with neurology, psychiatry or geriatrics
    • Monitoring for progression and complications
    • Intensive Dementia Care Management by a nurse practitioner supervised by a physician dementia specialist
    • Caregiver education through a community lecture series
    • Advance care planning intervention with a clinical ethics expert to focus on future care goals
    • Hospitalizations, when needed, on the Santa Monica-UCLA Geriatrics Special Care Unit or Geriatric Psychiatry unit at the Neuropsychiatric hospital
    • Referral, when appropriate, to community based organizations for services and materials

  • Providing immediate access 24/7, 365 days a year for assistance and advice to answer pressing questions in real time and manage crises to avoid Emergency Department utilization and hospitalization

Patient Eligibility

For your patient to be eligible for the UCLA Alzheimer's and Dementia Care Program, you must:

  • Be a UCLA Primary Care Physician, Neurologist, or Psychiatrist

  • Be willing to work with our Dementia Care Specialists to coordinate care for referred patients

How to Enroll Your Patient
