The UCLA Geriatrics Falls Clinic provides a multidisciplinary assessment of fall risk and personalized care plans for older adults. We focus on identifying the most common and modifiable causes of falls including strength and balance deficits, dangerous medications, vision impairment, unsafe footwear, blood pressure changes when getting, and cognition problems, as well as preventing complications of falls through optimizing bone health. What to Expect at Your Visit
Your visit includes:
- Check‑in & Intake: We review your pre-visit questionnaire, medications, and blood pressure.
- Screening: Vision screen, short cognitive screen, and fracture risk review.
- Physical Therapy Evaluation: Gait, strength, balance testing and assistive device assessment.
- Falls Risk Evaluation: A geriatrician (and supervised geriatric medicine fellow) performs a comprehensive assessment.
- Personalized Care Plan: On‑the‑spot education, orders, and referrals (e.g., physical therapy, ophthalmology, pharmacy, osteoporosis clinic).
- Summary & Follow‑up: We share a written care plan and provide recommendations to your primary care provider.
What to bring: Medication list/bottles, glasses, mobility devices, recent bone density results (if available), and completed forms.

- Will I need imaging or labs? Only if clinically indicated (e.g., bone density test, blood work if osteoporosis medications are considered).
- Do I need a referral? Many patients are referred by their primary care provider.
- Is there a home-assessment? You will complete a home safety checklist before you are seen and we can refer you to home physical therapy and in‑home safety evaluations, when appropriate.
- What programs help with ongoing exercise? The UCLA SAIL program (Staying Active and Independent Living) and community classes are common options.
