New Patients
Many insurance plans require a referral from a primary provider and/or "pre-authorizations." If your insurance plan requires this, you will need to make sure that authorizations or referrals are taken care of before you receive services at UCLA.
You may schedule services before receiving notice of authorization, but the service may be canceled if authorization is not approved by your insurance company.
New patients should have their doctor's medical records mailed or faxed to our office prior to the scheduled appointment. Please include the following:
- Cardiology Progress Notes
- Echocardiogram Reports (please send a digital copy of the echocardiogram on CD for most recent studies. DICOM format is preferred.)
- Cardiac MRI/CT
