How to Refer a Patient for Audiology Services
To refer a patient who has Medicare, please send a referral for Audiologic Evaluation via UCLA MyChart or send a paper referral via fax to 1-310-825-3344.
To refer an HMO patient, please submit an authorization for CPT codes 92557, 92567 and 92550 for adults.
For children ages 5+, please submit authorization for CPT codes 92588, 92587, 92582, 92555, 92556 and 92557.
For children under 5 years old, please submit authorization for CPT codes 92588, 92587, 92582, 92555, 92556 and 92579.
