Collaboration for the Good of All
Our street medicine program is built upon collaboration. We’re able to serve thousands of people each year through seamless coordination with other social service organizations, agencies and individuals who share a passion for improving the lives of people experiencing homelessness. This collaboration is essential to addressing the significant needs of the homeless population.
We are part of a network that establishes communication between street medicine providers, medical homes, housing and other supportive services. These collaborations allow us to expand our reach to serve the greatest number of people.
We cultivate relationships with shelters and other community partners so that we’re able to provide effective follow-up care, especially for people with chronic conditions. This continuity of care allows us to manage conditions more effectively, monitor patients’ health and deliver better treatments. Together with our partners, we provide consistent, comprehensive, trauma-informed care. We are grateful for our many collaborators, including:
