Physician & Surgeon License Renewal
- Application
- License Renewal Fee - $1206. There is no fee discount for renewal, regardless if you are going into practice or continuing training.
- The application must be completed online

- Review the Telephone Number and Email Address to ensure accuracy for your personal information. Avoid using institutional email or phone numbers.
- The Address of Record should be listed for the office of your future program or practice as this will be publicly available information. Please do not list our office as we do not hold or forward mail once you finish training with our program.
- Initial License Renewal (ILR) Requirement Form for Verification of Postgraduate Training:
- This is necessary for P&S licenses issued January 1, 2022, or later, such that the Board must verify completion of 36 months of Postgraduate training. Upon completing your Physician & Surgeon’s license renewal application, please complete the “Applicant Information” at the top of the ILR Form and submit via this form here rather than sending directly to Dr. Skinner. Our office will ensure completion and submission to the Board.
- For those renewing the P&S license: the license expiration date must be within the academic year in which reimbursement is sought for our program to be able to reimburse. However, for those continuing on to programs at UCLA, your application fee would be reimbursable by your fellowship program within the first 30-days of your fellowship start. You can expect your fellowship coordinator to reach out to you for your receipt upon your program start. For those not staying at UCLA and whose licensure expiration date falls outside of current IM training, application fees would not be eligible for reimbursement. However, if you are transitioning to a fellowship outside of UCLA, you could review reimbursement eligibility at your future institution.
- If you identify per the above information that you are eligible for reimbursement with our program, please submit your request here within 30-days of the incurred expense.
- Other fees related to completing your license application (e.g., transcript fees, LiveScan fees, etc) are intended for use from your education fund and are not reimbursable.
- The Song-Brown Family Physician Training Act is a voluntary fee and is not reimbursable.
