The UCLA Alzheimer's and Dementia Care program is designed to help patients and their families with the complex medical, behavioral and social needs of Alzheimer's disease and other types of dementia. Our Dementia Care Specialists are at the heart of the program and work with your UCLA primary care doctor, specialist, and/or referring UCLA physician to develop and implement a personalized care plan.
All patients are monitored at least annually to ensure that ongoing and emerging needs are met. In-person visits are covered by Medicare. Telephone calls and many other services (for example, support groups, educational programs, and referrals to community-based organizations) are at no cost to you.
The UCLA Alzheimer's and Dementia Care program strives to help every patient maintain independence to the highest degree possible, with dignity.
