The UCLA Alzheimer's and Dementia Care program is designed to help patients and their families with the complex medical, behavioral and social needs associated with Alzheimer's disease and other types of dementia. Our Dementia Care Specialists are at the heart of the program and work with your UCLA primary care doctor, specialist, and/or referring UCLA physician to develop and implement a personalized care plan.

The ADC Program's mission is to maximize the patient’s ability to function, while maintaining independence and dignity. Our team also works with families and caregivers to minimize caregiver strain and distress.

Our services:

  • A 90-minute Initial visit with one of our Dementia Care Specialists
  • A personalized care plan for each patient and caregiver
  • Follow-up care (telephone and in-person appointments) to ensure the plan is implemented or modified as needed
  • 24/7, 365 day a year access for caregivers who need assistance and advice in order to avoid Emergency Department visits and hospitalization
  • All patients receive an annual comprehensive assessment to ensure that ongoing and emerging needs are met. In-person visits, telemedicine video visits and scheduled telephone calls are covered by Medicare. The ADC Program also offers services that are at no cost to you (for example, support groups, educational programs, and referrals to community-based organizations).

Eligibility

  • All patients must have a diagnosis of dementia or a related condition

  • Must not reside in a nursing home or be enrolled in hospice

  • Must receive ongoing care from a UCLA Primary Care Physician, Neurologist, or Psychiatrist
