Become A Patient
There are two ways we can provide medical consultation – either in-person, teleconsult, or both.
An in-person consultation refers to the traditional, face-to-face interaction between the care team and patients in a doctor’s office.
A second opinion teleconsult is a virtual interaction between the care team and patients conducted via a secure video-conferencing platform. We also offer written second opinions which is a report written by a physician in response to medical records provided by the patient.
Purpose of Consultation/Patient Expectations
Please provide a brief explanation for the requested consultation. For example: Diagnosis confirmation, continued or alternative treatment options, access to clinical trials, etc.
Medical Records and Documents (Required for Both In-person and Teleconsultation Requests)
It’s important that we have your full medical history for our physician to make the most informed assessment of your condition and available treatment options. We may also ask you to provide your local physician contact info for UCLA physicians to better assess your medical condition.
Required medical records varies by condition, and may include:
- Diagnostic reports and images in DICOM format
Without the diagnostic imaging files, the physician will be unable to provide the most accurate medical advice and treatment plans.
- Laboratory results
- Procedure and Operative reports
- Full pathology reports (if possible, pathology slides)
- Genetic testing results
- Most recent physician progress notes (outlining history of illness, medical and surgical history, physical assessment, treatment history, current treatment, and medication list) at least within the last 3-6 months.)
All records must:
- Include patient name, date of birth, and the name and date of report.
- Be translated to English and be stamped and signed by an official translation service.
If you are unable to provide the necessary medical records for a specialist visit, we are happy to arrange a visit to a primary care physician for an initial assessment. This will then allow you to be referred to a specialist, as needed.
Non-medical information and documents required include:
- Patient’s full name
- Date of birth
- Home address
- Phone number
- Email address
- Passport copy
- Financial agreement form
- Patient confidentiality (HIPAA) form
Timely Communication
Primary Contact: Kindly designate one primary contact to coordinate with your assigned UCLA Health International Services Team. Ensure their contact information is current, and ensure they check all communications channels regularly.
Response Time: Please respond promptly to calls and emails from UCLA Health to help with scheduling, collecting documents, and pre-arrival preparations. This will minimize delays and allow you to receive care as quickly as possible.

Preparation for Visits In-person travel & logistics
Pre-Appointment Instructions: Plan travel and accommodation in advance, considering any specific recommendations from UCLA Health. Follow any pre-appointment instructions carefully, such as fasting requirements or other specific preparations.
Travel Timeframe: Indicate when you plan to travel to Los Angeles to help with appointment scheduling. Please be mindful that travel arrangements including obtaining a visa to the US can take some time to process and patients will need to plan accordingly to minimize rescheduling or delayed appointments. If you require a Visa Support Letter, payment must be received in full before the letter can be provided.
Teleconsult Logistics
UCLA’s International Telehealth Team will provide your appointment details including a secure video conference link via email, allowing you to join from the comfort of your home. Pending your state or country's legal requirements, a locally treating physician may need to be present with you on the call.
You will receive a copy of your consultation report by email once the teleconsultation is complete.
Patient Responsibilities
We understand life doesn’t always respect a calendar. Should you need to reschedule an appointment, please simply let us know at least 48 hours ahead by contacting the UCLA Health International Services Team. We're here to work together on your treatment plan, and we encourage open communication about what we recommend and what works best for you. Your healthcare decisions are important to us, and we're here to support you while ensuring you understand any potential impacts to your care.
Post-Care
It’s important to continue your care after you leave UCLA Health. We are here to support you during this transition, including knowing when and where to get further treatment and what you need to do at home to help with your care.
The UCLA Health International Services Team is available Monday through Friday from 7am to 6pm pacific standard time via phone, email, text message, WhatsApp, and WeChat. We are closed on major US holidays.
