Study the required training materials posted on this webpage (see the links provided below) prior to your check-in day. You must then print, sign and complete, and bring with you ALL forms to be collected on your check-in day. Any missing items from the list below will disqualify you as a volunteer applicant, and you will be instructed to re-apply at a later date. Allow yourself enough time to gather all the mandatory paperwork and immunizations. You will receive 5 hours of service credit for completing all paperwork.

IMPORTANT: You will need to generate a temporary I.D. to complete the following training modules if you do not have a Bruin number.           

PRINT ALL CERTIFICATES OF COMPLETION.  *UCLA students can use their BRUINCARD# as temp I.D. 

  • HIPAA Privacy and Information Security Training
  • CICARE Annual On-Line Training
  • Safe Patient Handling Training
  • Volunteer Services Training
  • Radiation Safety Training (Can only be completed on Internet Explorer)

Where will Check-In take place?

Check-in will take place in the volunteer office from 9:00am-4:00pm (closed during lunch time from 12:00 noon- 1:00pm).

What happens at Check-In ?

We collect and make sure all your paperwork is complete. No copies will be made of your paperwork including immunizations, items submitted will later be discarded; therefore, you should make copies for your own records. Make sure you have a valid government issued ID or Passport.

What Immunizations are required to volunteer?

Applicants MUST bring a copy (will not be returned) of their immunization records to check-in day (see page 12 in the Orientation Guide listed above). UCLA has very specific requirements for immunizations in order to volunteer. All documentation must be presented at check-in day.

Documentation required at volunteer check in day:

Measles, Mumps, and Rubella (MMR)

  • Medical documentation of TWO MMR vaccinations

                                       OR

  • Medical documentation of ONE BLOOD TITER indicating immunity to MMR
  • Disease history of MEASLES, MUMPS, and RUBELLA will not be accepted.

Varicella (Chickenpox)

  • Medical documentation of TWO VARICELLA vaccinations

                                        OR

  • Medical documentation of ONE BLOOD TITER indicating immunity to VARICELLA
  • Disease history of CHICKENPOX will not be accepted.

Tuberculosis (TB) Screening

If you have always tested negative for TB, please provide:

  • Medical documentation of a QuantiFERON-TB Gold (also known as QFT-G) blood test completed within the last 3 months.

If you have previously tested positive for TB, please provide:

  • Documented proof of a positive QuantiFERON-TB Gold (also known as QFT-G) blood test (documentation of positive TB from any date will be sufficient).

                                       AND

  • Medical documentation of a chest radiograph (chest x-ray) report within the last 3 months

Tetanus, Diphtheria, Pertussis Vaccine (Tdap)

  • Medical documentation of Tdap vaccination

                                   OR

  • Signed Tdap declination if unable to provide medical documentation.

Flu Vaccination

  • If volunteering October-April

Things to Remember

  • Bring with you all of the required paperwork, fully completed and signed to check-in day.
  • Have all required immunizations completed by check-in day.
