How to schedule an appointment with a menopause provider

What to expect at your first visit 

Prior to your appointment, you will be sent a comprehensive health questionnaire to help us better get to know you and your healthcare needs. This questionnaire will not only help us to address specific menopause concerns, but also help us determine if you are at risk for any other menopause-related health issues. We kindly ask that the questionnaire be completed at least 7 days prior to your visit so our providers have enough time to prepare. This questionnaire will take 30-45 minutes to complete.

At your first appointment, you will have a consultation with one of our certified menopause practitioners. At this visit, we will review your health history from the questionnaire and address your symptoms as they relate to menopause. Because the drop in hormone levels from menopause can affect many organs in your body, we will also assess your health as it relates specifically to the bone, breast, cognition, cardiovascular system, mood, sleep, and genitourinary systems.  We will discuss the options of treatment and work with you to come up with an individualized treatment plan.  If you need specialized care in any areas, we will connect you with our trusted team of interdisciplinary specialists. 

Follow-up appointments

We will schedule follow-up appointments with you as needed. We will also schedule a yearly comprehensive health assessment for you.
