Message from Department Chair, Deborah Krakow, MD
As we can finally view the unprecedented challenges of 2020 in our rearview mirror, we should look forward to 2021 with a sense of gratitude and hope. First, gratitude. Over most of 2020 our doctors, nurses and other health professionals worked in increasingly risky settings to deliver first class care and support to inpatients and outpatients. COVID impacts everyone - and doesn’t recognize a family expecting a new child or seeking assistance in planning a family, or a woman battling cancer. We continue to maintain a full schedule of services with very close attention to protecting our staff and patients from COVID-19 exposure. This work has required so much creative resolve and courage amidst so much uncertainty, and requires us all to endure changing schedules, workflows, and loss of many meaningful personal interactions. It is so difficult not to hug a crying patient or the parents of a new baby. COVID has forced us to sacrifice some of the elements of what makes us human, to ensure safety for all.
Second, hope. While we still have many tough days ahead, the start of 2021 brings us much hope for the future. The education and training of our future health professionals has continued strong during this difficult time. Vaccines whose efficacy seemed a dream last spring are now here and with them comes optimism that perhaps we might be turning a corner toward the pandemic’s end. And most important our healthcare system continues to hold steady through what is arguably one of the biggest challenges we have ever faced.
Our UCLA faculty and staff has been keenly focused on maintaining availability of services for our patients during this challenging time. In this issue of OBGYNews you will meet Dr. Christina Han, our new Division Director for Maternal-Fetal Medicine, and hear about recent successful efforts to streamline collaborations with experts across UCLA Health to improve care for women with complex pregnancies - from pre-conception to pregnancy to post-partum services. You will also read about new groundbreaking research in the lab of Dr. Sandra Orsulic, studying mechanisms in the microenvironment that contribute to cancer initiation and progression, and using that information to identify markers for early cancer detection and generate models for testing targeted therapies. And you’ll learn more about our newest OBGYN Fellowship in Minimally Invasive Gynecologic Surgery (MIGS), a two year fully accredited program that will begin in July with the 2021-2022 academic year.

Clinical Update
Advances in Maternal-Fetal Medicine Care
Caring for pregnant individuals with complex medical, obstetrical or fetal diagnoses is a challenge for providers, but early identification and application of a multidisciplinary care model can optimize outcomes.
Christina Han, MD, Associate Clinical Professor of Obstetrics and Gynecology and Division Director of Maternal-Fetal Medicine, is making it a priority for the high-risk obstetrics service to streamline collaborations with experts across the UCLA Health system to improve care for women with complex pregnancies, from pre-conception to pregnancy to the postpartum period. Dr. Han joined the faculty of UCLA’s Department of Obstetrics and Gynecology in October, and is currently building multi-disciplinary programs for cardio-obstetrics, diabetes in pregnancy, preconception care, and advanced prenatal diagnosis and genetic screening.
For those individuals with heart disease who desire pregnancy or are pregnant, UCLA relies on a collaborative approach to care with experts in maternal-fetal medicine, adult cardiology, pediatric cardiology, anesthesiology, neonatology, nursing, and mental health. These teams work together to provide specialized care in the most complicated scenarios, often with joint counseling sessions and multidisciplinary meetings. Yalda Afshar, MD, PhD, Assistant Professor and maternal-fetal medicine specialist in Obstetrics and Gynecology at UCLA, was recently an integral part of a large, multidisciplinary team of specialty providers coordinating care for a mother with pulmonary hypertension at 25-weeks gestation. UCLA’s top specialists from multiple departments came together to coordinate this extremely high-risk, but successful, undertaking to deliver a healthy baby in the intensive care unit. "When the family decided that they were going to continue with the pregnancy, we were ready," said Dr. Afshar. The story was featured on , and is just one example of the quality care provided by the UCLA high-risk obstetrics service.
Committed to being at the forefront of science, our maternal-fetal medicine specialists are also committed to advances in research and education. At the helm, Deborah Krakow, MD, Chair of Obstetrics and Gynecology and Professor of Obstetrics and Gynecology, Orthopedics and Human Genetics at UCLA, currently holds a PO1 and R01 aimed at identifying genes and molecular basis for recessive chondrodysplasias and musculoskeletal disorders.
Carla Janzen, MD, PhD, Associate Professor and Maternal-Fetal Medicine specialist in Obstetrics and Gynecology at UCLA, is part of a team of researchers spearheading a study to identify complications of pregnancy utilizing free-standing genetic material shed from the placenta into the mother’s blood. The team’s effort to advance knowledge about the placenta and maternal health will hopefully guide further development of tests to help diagnose pregnancy disorders earlier.
The Division of Maternal-Fetal Medicine is also actively involved in the University of California Fetal Consortium, a transdisciplinary collaboration among the five University of California medical centers aimed at optimizing maternal, fetal, and neonatal outcomes. Our experts and trainees in Maternal-Fetal Medicine and Pediatrics contribute to the Consortium by leading and participating in projects aimed at training, clinical research, and quality improvement initiatives.
Another priority for Dr. Han is to build an enhanced portfolio of clinical trials to improve outcomes for women with complex pregnancies. UCLA Obstetrics and Gynecology is currently recruiting pregnant subjects for prospective clinical trials in COVID-19, mental health disorders, and thromboprophylaxis, with additional clinical trials in the pipeline.
Coronavirus Outcomes in Mother-infant Pairs (COMP) is a prospective observational study evaluating clinical outcomes, viral shedding and development of immunity in mother-infant pairs affected by COVID-19. Rashmi Rao, MD, Health Sciences Clinical Assistant Professor, is co-principal investigator on this project. Dr. Rao is also leading the Heparin Dosing for Antepartum Hospitalizations (HEPDOSE) trial that aims to assess the effect standard dosing versus gestational age-based dosing of unfractionated heparin has on receipt of neuraxial anesthesia as well as to evaluate the pharmacokinetics and pharmacodynamics of unfractionated heparin in pregnancy.
PRIORITY (Pregnancy CoRonavirus Outcomes RegIsTrY) is a nationwide study of pregnant or recently pregnant women who are either under investigation for Coronavirus infection (COVID-19) or have been confirmed to have COVID-19 to help patients and doctors better understand how COVID-19 impacts pregnant women and their newborns. Dr. Afshar is a co-principal investigator on this study, along with a team of co-PIs from UCSF.
The New Moms Study, a prospective cohort study aiming to determine efficacy of standard versus technology-based mental health treatment during pregnancy, is led by Misty Richards, MD, MS, Assistant Clinical Professor of UCLA Obstetrics and Gynecology and UCLA Division of Child and Adolescent Psychiatry and Medical Director of Perinatal Psychiatry, and Dr. Rao, who is also the Obstetric Medical Director of Mental Health. This interdisciplinary team also heads the Maternal Outpatient Mental health Services (MOMS) Clinic serving the mental health needs of women during the perinatal period.
Dr. Han’s vision is “to expand on the strong foundations of our division to build a world-class Maternal-Fetal Medicine program that delivers compassionate and evidence-based patient care, performs cutting-edge innovative research, and trains a strong cohort of future Maternal-Fetal Medicine leaders.”
Education: UCLA Obstetrics and Gynecology Fellowship Programs
The UCLA Department of Obstetrics and Gynecology is committed to providing a robust and rigorous training ground for future physician-scientists. Our integrated and multidisciplinary fellowship programs are structured to train academically oriented obstetrics and gynecologists, innovative scientists and world leaders in women's health. We have six of the 7 ACOG-recognized subspecialty fellowship programs including, Complex Family Planning, Female Pelvic Medicine and Reconstructive Surgery, Gynecologic Oncology, Reproductive Endocrinology and Infertility, Maternal-Fetal Medicine and Minimally Invasive Gynecologic Surgery, plus a unique fellowship in Patient-Centered Outcomes Research Training in Urologic and Gynecologic Cancers. Our fellowship programs are led by nationally recognized leaders in these disciplines, preparing our fellows for distinguished careers in clinical practice, education, research, and public policy and service.
The Minimally Invasive Gynecologic Surgery (MIGS) Fellowship, our newest program, provides a 2-year comprehensive training curriculum in advanced gynecologic endoscopy and minimally invasive gynecologic surgery. Our program is fully accredited by the the American Association of Gynecologic Laparoscopists (AAGL), the leading association promoting minimally invasive gynecologic surgery among surgeons worldwide. The UCLA MIGS Fellowship is led by program director Tamara Grisales, M.D., Health Sciences Assistant Clinical Professor of Obstetrics and Gynecology, and associate program director Sukrant Mehta, M.D., Health Sciences Assistant Clinical Professor of Obstetrics and Gynecology. The fellowship will commence with the 2021-22 academic year.
Meet Christina Han, MD, the Newly Named Division Director for Maternal-Fetal Medicine
UCLA OBGYN is pleased to announce the hiring of Christina Han, MD, Associate Clinical Professor of Obstetrics and Gynecology and Division Director of
Maternal-Fetal Medicine. Dr. Han was most recently at the Center for Fetal Medicine and Women’s Ultrasound, a hybrid private-academic practice in Los Angeles. She received her medical degree from David Geffen School of Medicine at UCLA and fulfilled her residency in obstetrics and gynecology at Cedars-Sinai Medical Center. Dr. Han completed a maternal-fetal medicine fellowship at Yale University School of Medicine / Yale-New Haven Hospital, where she continued on as a Women’s Reproductive Health Research Scholar and Albert McKern Scholar for Perinatal Research.
Dr. Han is board-certified in maternal-fetal medicine and obstetrics and gynecology. Her clinical and research interests intersect and include advanced prenatal diagnosis and management of fetal syndromes, management of complex maternal comorbidities, with a focus on diabetes and endocrine disorders in pregnancy, and preconception optimization of health. She has published 43 peer-reviewed manuscripts and 38 invited reviews and editorials. Her clinical expertise will serve as a strong foundation for working with faculty to enhance the specialty care for individuals with high-risk pregnancies. Currently, she is building multi-disciplinary programs for cardio-obstetrics, diabetes in pregnancy and advanced prenatal diagnosis and genetic screening. She has also expanded the outpatient maternal-fetal medicine consultation service to allow in-depth discussions and anticipatory guidance for patients with maternal or fetal comorbidities.
Dr. Han is also a passionate educator. While at the Center for Fetal Medicine, a rotation site for the UCLA fellows, she became the program director for the UCLA Maternal-Fetal Medicine Fellowship. Since assuming the role in 2017, she has expanded the clinical, research and didactic missions of the fellowship. She is excited to leverage the continued advancement of the division to further enrich the experience for UCLA trainees.
Dr. Han is an active member of Society for Maternal-Fetal Medicine (SMFM), where she has field experience in leadership and program building. She served as chair of the Internal Communications committee, and is currently the vice chair of the Education committee. Her additional roles in SMFM include serving on the national COVID-19 task force, tasked with rapid development of guidelines and educational materials for care of the obstetric patient during the pandemic, and being a member of the Finance Committee for the Foundation for SMFM. She co-directs a well-attended “Rare maternal and fetal diseases” postgraduate course at the SMFM annual meeting, and produces the SMFM podcast series. Additionally, she is involved in other national societies, including International Society of Prenatal Diagnosis, where she served in the program committee in 2020, and the American College of Obstetrics and Gynecology, where she is currently on the Prenatal Care Expert Consensus Panel, tasked with an evidence-based redesign of prenatal care.
When considering the move to UCLA, her alma mater and academic home, Dr. Han said “I saw such potential for growth in the division, and the opportunity to carve out untapped clinical and research niches in the region. In conjunction with the superb faculty already here at UCLA, I plan to utilize my experience, expertise, and network to propel the UCLA maternal-fetal medicine division to become the world-class maternal-fetal medicine division that it will be.”
Dr. Han will be seeing patients in all of our maternal-fetal medicine offices. When not working, she enjoys spending with her two children and husband, hiking, swimming, indoor cycling, tending to her vermicompost, attempting to garden, and traveling (and eating) their way around the world. We are honored that Dr. Han is at UCLA and welcome her to the UCLA Health community.
Research Update
Meet Sandra Orsulic, PhD, prominent cancer biologist whose 20 years of research has helped to shape research modeling of early ovarian cancer development
In January the UCLA Department of Obstetrics and Gynecology welcomed Dr. Sandra Orsulic to our faculty. Dr. Orsulic is a prominent cancer biologist
whose work over the past 20 years has helped shape research modeling of early ovarian cancer development. After receiving her PhD from the University of North Carolina at Chapel Hill, Dr. Orsulic trained in the lab of Nobel Laureate Dr. Harold Varmus, and established her own laboratory research as Assistant Professor of Pathology at Harvard Medical School and Massachusetts General Hospital. For the last twelve years, Dr. Orsulic continued to grow her research portfolio at Cedars-Sinai Medical Center.
Research from Dr. Orsulic’s lab focuses on the development of mouse models in which ovarian cancers are induced and studied during its early stages of growth and metastatic progression. Her laboratory has generated multiple syngeneic mouse ovarian cancer cell lines with defined genetic alterations that recapitulate the development and pathophysiological manifestations of human ovarian cancer as well as the complexity of cancer-stroma interactions. Her team is using these models to determine the functional contributions of individual molecular pathways to cell transformation, immune response to cancer progression, recruitment of cancer-associated fibroblasts, and development of therapeutic resistance. Recent published efforts by the Orsulic lab are studying mechanisms by which the microenvironment contributes to cancer initiation and progression. This information is used to identify molecular markers for early cancer detection, and to generate functional models for testing targeted therapies.

UCLA faculty who have been recognized for excellence in clinical care, research and teaching
HONORS AND AWARDS:
UCLA pelvic medicine specialists Drs. Tamara Grisales, UCLA OBGYN Health Sciences Assistant Clinical Professor, Christopher Tarnay, UCLA OBGYN Health Sciences Associate Clinical Professor, Ja-Hong Kim, UCLA Urology Associate Professor, and the Center for Women's Pelvic Health group, were honored by UCLA Rehabilitation Services with a “Collaboration Award” during National Rehab Month. The FPMRS team was recognized for their support of the pelvic floor rehab program and ongoing collaboration.
Sanaz Memarzadeh, MD, PhD, UCLA OBGYN Professor, was recognized by the Los Angeles County Board of Supervisors for her research and awareness building for ovarian cancer. She also received a Certificate of Recognition from Mayor Lindsey Horvath and the West Hollywood City Council for outstanding service, commitment, and accomplishments in ovarian cancer research.
Tina Nguyen, MD, UCLA OBGYN Health Sciences Assistant Clinical Professor, was named an Educator for Excellence for the David Geffen School of Medicine at UCLA for the 2020-21 academic year. A group of 30 accomplished and dedicated faculty teachers with expertise in small group facilitation and an outstanding track-record of exceptional teaching were selected. This program and group of faculty will be a transformative new element in our medical student education program.
Molly Quinn, MD, UCLA OBGYN Health Sciences Assistant Clinical Professor, has been named as a CREST Scholar by the Clinical Research/Reproductive Scientist Training (CREST) Program. This training program is offered by the Eunice Kennedy Shriver National Institute of Child Health and Human Development (NICHD), the Clinical Research Training Program (CRTP) at Duke University, the American Society for Reproductive Medicine (ASRM), and the University of Colorado School of Medicine.
Aparna Sridhar, MD, MPH, UCLA OBGYN Health Sciences Associate Clinical Professor, has been appointed as the Vice Chair for applied Anatomy College for David Geffen School of Medicine at UCLA. Additionally, in January 2020 she will assume the role of Clerkship Chair for the department.
GRANTS:
Yalda Afshar, MD, PhD, Assistant Professor In-Residence of Obstetrics and Gynecology in the Division of Maternal Fetal Medicine, has been awarded five grants in recent months. She is the Principal Investigator (PI) / MPI for “PRIORITY: Supporting the Public Health Response to COVID-19” funded by the Center for Disease Control (CDC) Foundation. She is one of four multiple principal investigators who received a Gates Foundation Grant, as well as a Robert Wood Johnson Foundation 2020 Global Ideas Grant. Dr. Afshar also received an Iris Cantor Women's Health Center Pilot Award for “PRIORITY (Pregnancy CoRonavIrus Outcomes RegisTrY” and a David Geffen School of Medicine Seed Grant for her project “Improving prenatal planning and postnatal care in severe congenital heart disease: Prenatal cardiac magnetic resonance (CMR) imaging in-utero to improve care for high-risk pregnancies.”
Marta Epeidegui, PhD, UCLA OBGYN Assistant Professor-In-Residence, recently received three NIH awards. She is the project director for “The role of PDL1+ exosomes in AIDS-related lymphoma,” a supplemental award to the UCLA Health Jonsson Comprehensive Cancer Center parent grant for which Michael Teitell, MD, PhD is the Principal Investigator. For the project “The Natural History of AIDS in Homosexual Men,” she is the co-investigator. Dr. Epeidegui is also the director of the biomarker core for the AMC clinical trials, a cooperative consortium to promote clinical trials in AIDS-related cancers.
